Agreement terms

1. Programme and informed consent

I voluntarily choose the programme identified in this form and in my accepted order or invoice. Peak Brain UK provides QEEG recording and brain-data education, neurofeedback brain training, health and wellness coaching, and related programme support. The purchased programme description controls the included duration, sessions and fees.

Peak Brain teaches clients about measured brain and behaviour patterns and supports feedback-guided training. QEEG information is educational and is not a medical or mental-health diagnosis or treatment plan. Neurofeedback is a training process and conventional sensor-based neurofeedback does not send electricity into the brain.

I consent to the reasonable placement and removal of sensors, electrodes, caps, clips, gel or paste needed for the selected service, including necessary touch by trained staff or an approved helper. I may ask that a step stop at any time.

Peak Brain does not provide medical diagnosis, psychotherapy, treatment of a sleep disorder, prescribing or medication advice. I will use an appropriately qualified professional for those services and will not change medication on the basis of coaching or training alone. Peak Brain is not an emergency service; in an emergency I will call 999 or another appropriate emergency service.

2. Responsibilities, possible effects and alternatives

I will provide accurate information, follow setup and training instructions, use equipment only as directed, and promptly report significant or unexpected changes in sleep, mood, attention, arousal, behaviour, seizures, health or medication. For remote training I will maintain a reasonably private, safe space, suitable internet access and adult supervision where required.

Sensors, caps, clips, gel or paste may contact the scalp, ears, forehead or body and can cause temporary pressure, skin or sensory irritation. QEEG or training can be tiring. Some people report temporary headache, fatigue, dizziness, irritability, vivid dreams, sleep, emotional, arousal, attention or symptom changes. Remote work also carries hardware, internet and sensor-placement limitations. I will report persistent or concerning effects so training can be paused or adjusted and outside care recommended when appropriate.

Results vary and no benefit, outcome, timing or durability is promised. Alternatives include not participating, delaying, changing between available in-office and remote delivery, choosing another provider, or seeking medical or mental-health assessment and care. I may pause or withdraw consent to future services, subject to charges already earned and equipment-return duties.

3. Privacy and the authorised service team

Peak Brain assumes full confidentiality. Peak Brain UK and the small Peak Brain team enabled to support me may access the information reasonably needed for client service, QEEG review, protocol design, coaching, equipment, safety, payment and administration. Remote clients may receive a private, client-specific Slack channel. A UK or global team member is added only when supporting that client and access is removed when no longer needed.

Information remains within the authorised Peak Brain service team unless I direct outside sharing, a contracted provider is needed to operate or deliver the service, or a narrow legal or safety ground applies. Appropriate processor and international-transfer safeguards are used where required. Peak Brain does not sell client records or use identifiable brain, health or coaching information for unrelated advertising.

The Peak Brain UK Privacy Notice, last updated 20 August 2026, explains the controller, information collected, purposes, lawful bases, processors, international transfers, retention, security and individual rights. I authorise Peak Brain to email my signed copy to the address I provide and understand that ordinary email carries some privacy risk.

4. Fees, cancellation and starting early

Peak Brain provides a full refund if I give clear cancellation notice within 14 calendar days after purchase; I do not need to give a reason. I may cancel by a clear statement emailed to info@peakbrain.co.uk. This contractual full-refund promise applies even if I asked Peak Brain to begin services during that period and does not limit a statutory cancellation right or other mandatory remedy.

After the initial 14 days, if I end a fixed-term programme early, Peak Brain will refund the unused portion of the programme fee, prorated by the time elapsed from the agreed programme start date through the effective cancellation date. For example, ending a two-month programme after one month ordinarily produces a 50% refund of that programme fee. A fully delivered one-time service has no unused programme time. No refund is based on achieving or not achieving a desired outcome.

If Peak Brain issued a loan kit, every return duty survives cancellation. Any voluntary prorated refund after the initial 14-day period will be processed after the complete kit is returned in accordance with Peak Brain’s instructions. Peak Brain may also await return before processing another voluntary refund, except where applicable law requires earlier payment. Ordinary wear is accepted; separately valid amounts for missing or damaged components may be deducted, but Peak Brain will not recover twice for the same loss.

Other billing, scheduling, rescheduling and pause terms appear in the accepted checkout, order, invoice or written enrolment material. Changing between available London and remote neurofeedback does not change the purchased programme price or duration. Nothing in this agreement removes a mandatory UK consumer protection.

5. London and remote delivery; loaned equipment

QEEG brain mapping is performed in office at Peak Brain Kensington, London; QEEG equipment does not leave the office. Neurofeedback may be completed in the London office on Monday, Wednesday or Friday, subject to booking availability, or remotely with support from Peak Brain’s global team. I may ask to change between those arrangements during the programme. Peak Brain will coordinate the change and confirm when a loan kit is issued or due.

For in-office neurofeedback, equipment remains onsite and I have no return obligation. For remote training with my own compatible equipment, I remain responsible for ownership, maintenance, safe operation and licences that I supply, and Peak Brain’s loan-kit return charges do not apply. Peak Brain-provided software, credentials, protocols and licences remain subject to their ownership and use restrictions.

For remote training with Peak Brain equipment, the inventory or dispatch record identifies the neurofeedback kit actually issued. The kit remains Peak Brain property and is loaned only while I am in an active programme. It becomes due when the programme is completed, paused, cancelled or terminated, when I change to in-office or owned-equipment training, or when Peak Brain requests return for safety or support reasons, unless Peak Brain confirms an extension in writing.

I will keep the kit secure, use it only as instructed, report loss, theft, damage or malfunction promptly, and return all listed electronics, sensors, cables, accessories, cases and other components using Peak Brain’s instructions and prepaid label when provided. Ordinary wear from proper use is expected.

Peak Brain provides a 14-calendar-day grace period after the confirmed return due date and written return instructions. After that grace period, Peak Brain may charge £1,000 for each started 30-day period of continued possession, up to £4,000 for the outstanding neurofeedback kit. Billing stops when the carrier records its first acceptance scan or Peak Brain confirms hand delivery. These amounts do not transfer ownership and are intended to address continued possession, loss of use, recovery, electronics and associated software licensing—not to punish. Peak Brain will not recover twice for the same loss and will reduce or credit an amount if keeping it would materially exceed Peak Brain’s reasonable documented loss. Mandatory consumer protections remain unaffected.

6. Electronic signature and governing terms

By typing my full legal name and submitting this form, I intend to sign electronically and agree that the electronic record may be used as evidence of this agreement. Peak Brain will send me a durable copy and may retain the date, time, version, browser information, IP address and a record-integrity hash.

This agreement and the accepted purchase or enrolment materials form the agreement on these subjects. If a term is unenforceable, the remainder continues so far as permitted. The laws of England and Wales apply, but no term removes a mandatory protection of the country in which I ordinarily live or a right that cannot lawfully be waived.

Consent and signature

By accepting the agreement below, I confirm:

  • I understand the educational and coaching scope and that Peak Brain does not diagnose, provide psychotherapy, treat sleep disorders, prescribe or advise medication changes.
  • I understand the possible effects, limitations, alternatives and lack of guaranteed outcomes, and I will report concerning changes.
  • I understand how the small authorised UK and global Peak Brain team may support me and how the Privacy Notice applies.
  • I understand the 14-day full-refund period, the later time-prorated refund, the electronic signature and, if a remote kit is issued, that a voluntary prorated refund is processed after return and the separate non-return rules still apply.
  • I have had an opportunity to ask questions and voluntarily choose to participate.

The typed name must match the signer name above. The server records the actual UK signing date and time.
